arbias case management services provide a person centred approach to assist clients to achieve their goals enabling them to participate within their community and maximise their level of independence.
This is achieved through an initial neurobehavioural assessment (outlined below) which provides the arbias case manager the opportunity to understand the individuals’ needs due to the impact of their ABI so that they can work together to set goals.
The case manager then assists in the coordination of appropriate services for the individual to enable them to achieve greater independence and community integration, whilst monitoring and reviewing their goals.
As part of the ADHC funded service, an individual will receive 12 weeks of specialised case management support by arbias with the plan that other services can provide ongoing support as required with the secondary consultancy support of arbias as required.
