| How to Make a Referral |

Referrals must be made using the ABIAU referral form. Referrals can be made by a person, family member, social worker, GP, treating professional or any other worker involved with the person (please ensure that person gives consent prior to referral being made). It is especially important for the person making the referral to clearly state what they and the client want to find out from the assessment. This is called the “referral question”. A clear referral question provides the neuropsychologist with a focus for the assessment and enables the testing and report to be targeted to address the key issue(s) of concern. The ABIAU has a waiting list for neuropsychological assessments. All referrals are initially reviewed by the Manager: Assessment Services . The person making the referral will then be notified when it is anticipated an appointment will be scheduled. In some cases the person making the referral may be asked to provide additional information (e.g. medical reports) or to clarify some of the information on the referral form before the client can be placed on the waiting list.
